The California Chronic Care Coalition is a statewide alliance of consumer health organizations and provider groups that advocates for people with chronic and rare conditions in California. Its mission is to improve access to affordable, high-quality, coordinated health care, with an emphasis on prevention, early diagnosis, chronic care management, and patient-centered care.

Primarily supported by charitable donations, organizational memberships, grants, and contributions from supporters and partner organizations.
More than 30 consumer health organizations and provider groups; leadership includes Liz Helms, President and CEO, and members from organizations such as the California Pharmacists Association and the American Diabetes Association.
